Para este tipo de datos, deberá implementar tanto el UNPIVOT y luego el PIVOT Funciones de SQL Server. El UNPIVOT toma sus datos de las múltiples columnas y los coloca en dos columnas y luego aplica el PIVOT para transformar los datos de nuevo en columnas.

Si conoce todos los valores que desea transformar, puede codificarlos, de forma similar a esto:
